Come on in ~ you will be greeted with a smile by Nicole, our Front Desk C.A.
She will have you fill out some general paperwork so we can get you started on the road to health.
If you would like to fill out your paperwork before you come in, click on the "new patient forms" tab to print the initial intake forms. Then all you have to do is remember to bring them with you when you come.
After Dr. K has a look at your paperwork, he will introduce himself & get started on your consult and exam.
